Your First Appointment

Patient Forms

If you’re a new patient, please complete the following forms and bring them to your first appointment.

We welcome the privilege of participating in your rheumatology care. Our staff strives to make your experience at our practice a pleasurable one. During your initial appointment you will establish care with one of our Board-Certified Rheumatologists or a Rheumatology trained Advanced Practice Providers (APP).

They will be your main providers and will service your needs throughout the duration of care with our practice. The Physician Assistant and Nurse Practitioners work hand-in-hand with Dr. Alper and Dr. Alvarez to provide comprehensive care to our patients and are qualified to service our patient population. On occasion, you may be asked to see a provider within the practice to accommodate for vacations or unforeseen circumstances. However, all providers will have the information required to provide good continuity of care.

Prior to scheduling your initial appointment with one of our providers we would like to understand the reason for your referral.

Insurance We Accept

Please remember to bring the following items to your appointment:

  • Insurance ID Cards
  • Photo ID (Driver’s License or Passport)

Co-payments , deductibles and account balances are collected upon check in. For self-pay patients, payment will be collected upon check in. For your convenience we accept cash, checks and all major credit cards.

We ask that you arrive 30 minutes prior to your new patient appointment time  and 15 minutes for established patients to avoid cancellations.

As the needs of our community continue to grow, we look into continue to expand our affiliations with different payors. At this time, we are not affiliated with Medicaid and are not able to accept Medicaid patients as primary or secondary insurance.  If you do not see your insurance listed below or have questions about your coverage, please call our office at (239)-262-6550.

  • United Healthcare (Excluding United Healthcare Medicare Complete)

Pre-Appointment Information

Prior to your appointment you will receive an email invitation to pre- register in our patient portal. During your initial access, you will be prompted to create a log in and password. Please keep this information stored safely as it will allow you to access your summary of care and labs moving forward. Please complete the pre- registration process under the appointment time prior to each appointment to allow for shorter waiting times. We value your time and will make every effort to stay on schedule and avoid unnecessary waiting. If you are over 7 minutes late, we may need to cancel, delay or accommodate with a different provider.
